We ask that patients provide proof of insurance, insurance information and any co-payments at each visit.  Please consult your insurance provider regarding specific coverage questions and payment submission procedures.  We participate in various insurance plans and suggest you call our office staff prior to your appointment to verify plans which are currently accepted.

For those patients who do not participate in an insurance plan, we require payment at the time of service.  We accept cash, personal checks, Visa and MasterCard.
